The Prayer to be reborn in Sukhavati (Tib. Dewachen) is a short prayer that is often chanted at the end of session, particularly with the Chenrezig practice.

One of the ways to have a very favorable birth is to develop the strong intention that when you die you will go to the pure land of Chenrezig (Dewachen) and there you will be able to pactice the Mahayana Dharma constantly with no impediments. Later, you will be able to leave Dewachen and go to another pure land or return to samsara.
